We'd like to share a few guidelines so that we can serve you best.


Insurance: Family Functional Medicine does not accept insurance or file claims with your insurance provider. However, you can submit your bill to your insurance carrier for them to consider covering as an out of network provider.

Dr. Ingram is not a Medicare or Medicaid provider. If you have Medicare you will be asked to sign an additional form acknowledging that Medicare will not be responsible for your bill.

Children: Dr. Ingram will see children. For those under 2 years of age, we suggest that they have a pediatrician who sees them for the regular Well Child checks to monitor growth and development. 

Dr. Ingram supports the parent’s decision on vaccinations and we will have discussions on safety and importance of immunization.

Payment: Please note that we are a direct-pay clinic. We request a credit card number to be held prior to initial visits, phone consults and telemedicine visits. We accept cash, checks and credit cards.

A $200 credit card deposit is required at the initial arrangement of your appointment. Your deposit will hold your appointment slot. We accept cash, checks and credit cards. After initial deposit has been made, all new patients will receive an email with a link to set up a personal account on the Family Functional Medicine Patient Portal. All new patients are required to create their own portal account, complete the Medical History Forms (7 total) and add all prescription medications & Supplements into the system.

Please note: All deposits will be forfeited if required Medical History has not been completed within 2 business days prior to that first appointment. If you have any questions, please contact our admin team at: 336-365-6010.

Pre-Paid packages provide greater value and have expiration dates. Any appointments not used before that date will not be refunded.

If a check is returned for insufficient funds or rejected for any reason you will be responsible for the charges plus a processing fee of $40.00.

Scent Free Zone:

Because we treat patients with allergies, asthma and chemical sensitivities, we request that you avoid wearing perfume, cologne or scented products on your appointment day.

Cancellation of Appointments:

We understand that unexpected changes of schedule occur. There is a 48 hour/2 business day cancellation policy. We reserve the right to charge your credit card on file for the full amount of the missed visit for a follow up appointment and half the amount for a new patient appointment if it is not cancelled or rescheduled 48 hours (2 business days) prior to your appointment.
